Storing average value in database or global scope?
Caching the average rating seems to be a good idea in general in case you don't retrieve all reviews from the database anyway. I would then store the average rating together with all the other information about the product in the database.

How to organize my test functions?
It is very common to group your tests as methods of a specific class. Typically these methods all test the behaviour of a specific class (the System Under Test). If ClassA is the SUT you would then call the class holding all the tests ClassATest accordingly.
